Nursing Home LTHHC

This unit provides guidance to applicants and potential applicants inquiring of prospective medical eligibility and/or availability of long-term care facilities. Staff review applications to:

• Determine financial eligibility for the spousal impoverishment program and the Long Term Home Health Care Program (LTHHCP)
• Determine financial eligibility for placement or continued residence with adult care facilities, and  
• Assist in arranging for proper placement and location of beds with regard to adult care facilities.
