Sullivan County Public Health Services is ready to provide you with
many of the health services you need, right in your own home. Whether the health care you need is short or long term, basic or
more complex, home health care is something to be considered. Perhaps
you are about to return home after a stay in the hospital. Wouldn't you
feel more comfortable knowing that a dedicated group of health
professionals are available to help you along the road to recovery?
Sullivan County Public Health Services is the sole Certified Home Health Agency in the county. We provide a wide range of health and social services to persons recovering from an illness or injury, or persons who are disabled and/or chronically ill.
Our services include nursing, therapeutic treatments (physical, speech, occupational therapy) and home health aides. We accept commercial insurance, Medicaid, Medicare, and sliding scale fees.
Telehealth monitoring is available for home care clients. This system utilizes the “Health Buddy” appliance that allows the patient to electronically transmit blood pressure readings, pulse, and weight measurements daily for the Public Health nurse to review. The Health Buddy provides health teaching on a daily basis.
REFERRALS ARE ACCEPTED FROM: Self/Family, Physicians, Nursing Homes, Hospital Discharge Planning, Rehabilitation Centers and Community Agencies.
