The Certified Home Health Agency at Sullivan County Public Health Services is ready to provide you with many of the health services you need, right in your own home.
NEW! TELEHEALTH Monitoring is available for home care clients. This system utilizes the “Health Buddy” appliance that allows the patient to electronically transmit blood pressure readings, pulse, and weight measurements daily for the Public Health nurse to review. The Health Buddy provides health teaching on a daily basis.

Sullivan County Rural Health Network
The mission of the Sullivan County Rural Health Network is to collaborate to improve the health status of Sullivan County residents by increasing access to coordinated, cost effective health care services. Funding is provided by the New York State Department of Health, Office of Rural Health.
SULLIVAN NYCONNECTS Choices for Long Term Care
Provides consumers and their caregivers with comprehensive and unbiased information about long term care options in Sullivan County. This program serves the elderly, children with physical disabilities, and adults with physical disabilities -- regardless of payment source -- through a Single Point of Entry for information and assistance.
