When considering health care for yourself and your family, you need to be informed about your options. Hospitals and nursing home facilities are not always the solution to your health care needs. You have a choice. Have you considered that home health care may be the answer to your particular needs? The Certified Home Health Agency at Sullivan County Public Health Services is ready to provide you with many of the health services you need, right in your own home.
NEW! TELEHEALTH Monitoring is available for home care clients. This system utilizes the “Health Buddy” appliance that allows the patient to electronically transmit blood pressure readings, pulse, and weight measurements daily for the Public Health nurse to review. The Health Buddy provides health teaching on a daily basis. The goal is to reduce re-hospitalizations by supplementing home care nurse visits with an electronic health buddy. This allows for more frequent monitoring of patients with chronic conditions .
